Health Net Foundation, Inc., the charitable arm of Health Net Inc. (NYSE: HNT) has announced funding grants to help bolster home health care delivery in Los Angeles County.
Through $150,000 in grant funding provided to Northeast Valley Health Corporation and 27 additional Federally Qualified Health Centers (FQHCs) and community clinics, a new in-home health care program, Project CARES will launch.
The program is based on a home health assessment and care coordination program for seniors, those who are chronically ill, and those dually eligible for both Medicare and Medi-Cal benefits.
“This grant is expected to enhance delivery of health care services,” said Patricia Clarey, president of the Health Net Foundation. “Project CARES is an innovative collaboration that will help provide community-based, comprehensive care coordination for complex patient needs.”
The project goals include providing continuous care through home assessments and visits; linking patients with prevention and wellness services; and supporting safety net providers in capturing a patient’s history of chronic health problems to facilitate care planning, according to Health Net.
“This gift enables us to expand our reach and our services to focus even more on the medically underserved residents of Los Angeles County,” said Kim Wyard, chief executive officer of Northeast Valley Health Corporation, one of the nation’s largest community health centers. “The Health Net Foundation understands the challenging health care issues that exist in the communities we serve, and we are delighted to have its help in reaching out to our most vulnerable patients.”
