Transactions: Community Based Care Buys Your Home Court Advantage; WellSky Acquires Experience Care

Community Based Care purchases Your Home Court Advantage

Community Based Care (CBC) recently acquired Your Home Court Advantage to expand its veteran-focused services lines and its footprint.

“The transaction is a reflection of both the company and the marketplace,” Cory Mertz, managing partner at Mertz Taggart, told Home Health Care News in an email. “Despite recent headwinds, there is still strong demand for growing, profitable home care companies, mostly from private equity- and family office-backed portfolio companies.”

CBC is a provider of home care services and iIntellectual and developmental disabilities supportive services.

On its end, Your Home Court Advantage is a home care company that operates across 26 counties in Northeast Ohio. The company specializes in providing personal care services to veterans and other seniors through Ohio’s PASSPORT Medicaid waiver program.

“We are thrilled to welcome Your Home Court Advantage into the Community Based Care family of providers,” CBC CEO Mike Kotzen said in a press release. “This acquisition aligns perfectly with our mission to provide high quality, personalized care to seniors and our deserving veterans. Together, we will build on our shared commitment to improving the lives of those who have served our country and the generations before us.”

As part of the transaction, CBC will take on all operations, contracts and the staff of Your Home Court Advantage in a stock transaction. Your Home Court Advantage’s current team will remain intact.

WellSky buys Experience Care

WellSky — a post-acute technology company — acquired Experience Care.

Overland Park, Kansas-based WellSky utilizes software and analytics to help providers across the continuum achieve better outcomes at lower costs.

Experience Care is a technology company that also provides software solutions for post-acute care companies. The company has 150 long-term care clients with 850 facilities.

With the addition of Experience Care, WellSky becomes a well-resourced and scalable intelligent care management partner for long-term care providers.”

The companies believe that their organizations, together, are a natural fit. The companies will continue to invest in care coordination, analytics and interoperability in the future.

Family Resource Home Care acquires Companion Care

Family Resource Home Care has purchased Companion Care Inc.

“We are thrilled to announce our acquisition of Companion Care, a successful home care agency in the Puget Sound region,” Family Resource CEO Jeff Wiberg said in a statement. “Their exceptional track record and dedication to providing outstanding care aligns perfectly with our values. We look forward to warmly welcoming Companion Care into our family and together, we will continue to deliver industry best services to even more individuals and families in need within the community.”

Family Resource Home Care is one of the largest independent personal care companies in the Pacific Northwest. The company’s services include personal care, medication management, meal preparation, housekeeping, companionship, dementia support and end-of-life, transitional and respite care.

Over the past few years, Family Resource Home Care has continued to expand its geographic footprint to increase market share in the Pacific Northwest. With the completion of the deal, the company has seven locations in the Puget Sound region and 29 across Idaho, Oregon and Washington.

Colorado VNA and GoldLeaf HomeHealth agree to combine

The Colorado Visiting Nurse Association (Colorado VNA) has agreed to combine with GoldLeaf HomeHealth. The two companies will combine their staff and patients into Colorado VNA.

This expanded Colorado VNA will continue to operate as a mission-driven, nonprofit, home health, hospice, wellness and CAPABLE care agency. The combined companies will be able to serve more patients in need of care services.

“This move will strengthen Colorado VNA, benefiting patients, employees as well as referring providers and facilities,” Tim Bowen, president and CEO of the Care Synergy network, said in a press release. “The transition will be smooth while expanding the continuum of care offered throughout Colorado VNA’s service area.”

As part of the agreement, Julie Nunley will remain president of Colorado VNA.

“I welcome the GoldLeaf HomeHealth staff, their patients, and families,” Nunley said in the press release. “They represent a strong commitment to providing the very best home health and wellness care to every patient and family member they have served. That aligns well with the legacy of Colorado VNA.”

David Geras, the CEO of GoldLeaf HomeHealth, will also join Colorado VNA.

“Colorado VNA has a distinguished community legacy of more than 130 years which made the decision to join them a logical next step for GoldLeaf HomeHealth,” he said in the press statement. “This partnership will allow us to continue to provide our patients and their families with clinical expertise and the breadth and depth of care offered by Colorado VNA. I am excited that the highly talented GoldLeaf HomeHealth staff and I will be joining Colorado VNA, one of the largest home health, hospice, wellness and CAPABLE agencies in the state.”
