SCL Health System and its joint venture partner Univita Health recently announced the acquisition of Denver-based All Care Home Health LLC for an undisclosed amount.
All Care is a home health care company that provides skilled nursing, physical and occupational therapy, and social work services in the Denver metro area. The acquisition marks an execution of the SCL Health System and Univita Health joint venture’s strategy to expand health care services into the home.
The addition of new integrated acute and post-acute care delivery model will be introduced at three of SCL’s Denver area hospitals, Exempla Good Samaritan Medical Center, Exempla Saint Joseph Hospital, and Exempla Lutheran Medical Center. It will also be introduced to the Exempla Physician Network.
By acquiring the home health care company, the health system joint venture can provide coordinated care from the time patients are admitted to one of the system’s participating hospitals all the way through the post-acute, at-home care phase. Patients recovering at home will be visited by All Care workers who will perform assessments and oversee all components of care including medication compliance, diet, physical therapy, and emotional well-being.
The extension of care beyond initial hospital stays has been proven to reduce the number of readmissions attributed to health complications and therefore save money, says the joint venture.
All Care will continue to serve its current patients, physicians, and hospitals.
“All Care’s mission is well-aligned with the joint venture’s plan to establish an integrated care delivery model,” said Dr. Rick Lopes, senior vice president of health networks at SCL Health System, in a statement. “Our goal is to establish a more cost-effective care model that expands the continuum of care from hospital to home, reducing the length of hospitalizations, decreasing unnecessary readmissions and improving the overall patient experience.”
